Job summary

Avantara Norton invites a dedicated Social Services Director to join our skilled nursing team in Sioux Falls, SD. You will ensure residents' social and emotional needs are met, coordinate discharges, and participate in care conferences to uphold our high standard of care.

Minimum requirements include a bachelor’s degree in Social Work or similar field and two years of Social Worker experience, preferably in healthcare; a graduate degree with five years of related experience is preferable.

Qualifications

• Bachelor’s degree in Social Work or similar field.
• Two years of experience in a Social Worker role, preferably in a healthcare setting.
• Graduate degree in Social Work or similar field with five years of related experience preferable.

Responsibilities

• Prepare residents for their future and ensure their social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Support quality of care and admission processes in a Skilled Nursing Facility.

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Norton by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
To be eligible for consideration applicants should have:
As a minimum, a bachelor’s degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
