Job summary

Avantara Norton is seeking a Social Services Director to join our caring team in skilled nursing. You will prepare residents for their future and ensure their social and emotional needs are met during their stay, coordinating discharges and participating in resident care conferences.

The ideal candidate holds a bachelor's degree in Social Work or similar field with two years of experience in a Social Worker role, preferably in healthcare.

Qualifications

• Bachelor’s degree in Social Work or similar field.
• Two years of experience in a Social Worker role, preferably healthcare.
• Social Services Director experience in Skilled Nursing Facility preferred.
• Graduate degree in Social Work or similar field preferred.

Responsibilities

• Prepare residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Provide knowledge, skills, and attention to detail to ensure residents are comfortable and safe.
