Job summary

Avantara Mountain View is seeking a Social Services Designee to join our caring team. You will support residents by addressing social and emotional needs, coordinating discharges and contributing to care conferences.

Requirements include a high school diploma or equivalent, two years in a Social Worker role (preferably in healthcare), and a bachelor’s degree in Social Work or related field with five years of related experience being a plus.

Qualifications

• Minimum high school diploma or equivalent.
• Two years of experience in a Social Worker role, preferably in healthcare.
• Bachelor’s degree in Social work or a related field is preferable, and five years of related experience is a plus.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay.

As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Mountain View by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
