Job summary

Signature HealthCARE of Greeneville is seeking a Social Services Director to identify and meet residents' social, emotional, and psychological needs, and to support discharge planning.

You will lead documentation to comply with state and federal policies, participate in care planning, and serve as a resource for residents and families.

A Bachelor's or Master's in Social Work or Psychology and at least 3 years of social services experience are required.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• Minimum 3+ years of experience in Social Services, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
