Job summary

Signature HealthCARE, LLC is seeking a Social Services Director to identify and meet residents’ social, emotional, and psychological needs during their stay and assist in discharge planning. You will collaborate with care teams to support residents and families and ensure quality outcomes.

The role requires a Bachelor’s or Master’s in Social Work or Psychology, plus 3+ years in social services within a healthcare setting, and strong professionalism with confidentiality.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

Overview
