Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center in Bardstown, KY seeks a Social Services Director to identify and meet residents' social, emotional, and psychological needs. You will help plan for discharge and participate in care planning, working with families to address issues and coordinate services.

The role requires a Bachelor’s or Master’s in Social Work or Psychology and 3+ years in social services, preferably in healthcare, with a strong commitment to confidentiality and

Qualifications

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ years experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and provide for each resident’s social, emotional and psychological needs during stay at the facility and assist in discharge planning.
• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.
• Serve as a general source of information for residents and their families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

What You Need To Make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Salary Range

Up to USD $27.09/Hr.
