Job summary

Signature HealthCARE of Greeneville is seeking a Social Service Staff to support residents' social, emotional and psychological needs and assist in discharge planning. You will collaborate with residents and families, prepare timely documentation, and help the Social Services Director develop resident-centered care plans in our 154-bed facility, located in Greeneville, Tennessee.

This role is part of a compassionate team in an award-winning organization committed to person-directed care.

Qualifications

• Minimum 1 year of social work experience, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.
• Effective verbal and written communication skills.

Responsibilities

• Assist residents and families with social, emotional, and family issues.
• Assist in providing timely and accurate documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.
• Serve as a general information source for residents and their families.

Overview

As a Social Service Staff you will assist the Social Services Director/Coordinator in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

• Assist in prividing timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.
• Serve as a general source of information for residents and their families

What you Need to make a Difference

• At least one (1) year of experience working in social work, preferably in healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
• Effective Verbal and Written communication skills.
