Signature HealthCARE is seeking a Social Services Director at North Hardin Rehab & Wellness Center in Radcliff, KY. The role focuses on identifying residents’ social, emotional, and psychological needs and guiding their post-stay discharge planning within a 164-bed facility.

Candidates should hold a Bachelor’s or Master’s in Social Work or Psychology, have 3+ years in Social Services (healthcare preferred), and demonstrate professionalism and confidentiality.

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years experience in Social Services, with individuals, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation to comply with policies.
• Participate in resident care planning.
• Participate in development of discharge planning and prepare resident for transition.
• Serve as a general source of information for residents and families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

• Assist residents and their families with social, emotional, and family issues.


• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.


• Participate in resident care planning.


• Participate in the development of Discharge planning and prepare resident for the transition.


• Serve as a general source of information for residents and their families.

What you Need to make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.


• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.


• High level of professionalism and ability to maintain confidentiality.
