Job summary

Signature HealthCARE is seeking a Social Services Director to identify and address residents' social, emotional, and psychological needs while supporting discharge planning and ongoing development during stays at the Greeneville facility.

You will coordinate family support, documentation compliance with state and federal policies, and participate in care planning. A Bachelor’s or Master’s in Social Work or Psychology and 3+ years in social services in healthcare are required.

Qualifications

• Bachelor’s or Master’s degree in Social Work or Psychology.
• Minimum 3 years of experience in social services within a healthcare setting.
• Strong professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

What You Need To Make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
