Job summary

Under the Social Services Director, you will support admissions, discharge planning, and family councils, while maintaining confidentiality and coordinating with a multidisciplinary team to deliver person-centered care.

Qualifications

• High school diploma or equivalent required.
• Ability to work with elderly residents who may be ill, disabled, or emotionally upset.
• Organize, prioritize, and meet deadlines.
• Maintain confidentiality and discretion.
• Knowledge of nursing practices, terminology, and long-term care regulations.

Responsibilities

• Assist residents and families with adjustments to placement and plan of care.
• Help with treatment wishes and transitions within the facility.
• Participate in resident and family council meetings; support admissions and discharge planning.
