Ballardhrc is seeking a Social Services Designee in Seattle to support residents and families as they adjust to placement and the care plan. You will assist with admission, discharge planning, and ensure alignment with federal/state regulations while maintaining confidentiality.
Under the Social Services Director, you will help residents with treatment wishes, support transitions, and participate in family meetings to foster a person-centered care approach.

Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures. You will assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility. You will also participate in resident and family council meetings as requested, and support Admissions and assist with discharge planning services.
