Job summary

Santa Monica Healthcare Center is seeking a Social Services designee to support the Social Services Department. You will assist with care plans, psychosocial assessments, Medi-Cal applications, transportation needs, and discharge planning, coordinating closely with Case Managers.

This role requires documenting in line with policy and regulations, leading family care plan meetings, addressing grievances, and making room changes as needed, with compensation starting at $23.00 per hour depending on

Qualifications

• Social Services designee approved program or certificate required.

Responsibilities

• Assist in completion of care plans and psychosocial assessments, Medi-Cal applications, transportation needs, discharge plans, psychotropic monitoring of residents (assisting Nursing staff with meetings), and coordination of care with Case Managers.
• Completes required forms and documentation in accordance with company policy and state and/or federal regulations.
• Coordinates care plan meetings with families on admission and quarterly thereafter.
• Helps resolve grievances/concerns of Resident and/or family.
• Makes room changes when necessary.
