Job summary

Windsor Country Drive in Fremont, CA is seeking a Social Services Designee to join our resident-centered care team. Under the direction of the Social Services Director, you will assist residents and families with adjustment to placement, treatment plans, and discharge planning to support the highest practicable level of functioning.

We value confidentiality, timely communication, and teamwork. Required high school diploma; ability to work with elderly residents; knowledge of long-term care

Qualifications

• High school diploma or equivalent is required.
• Ability to work with physically and mentally ill, disabled and emotionally upset elderly individuals.
• Ability to organize, prioritize, and meet deadlines.
• Ability to maintain confidentiality and be discreet.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
• Experience with geriatric population is a plus.
• Sufficient computer skills to manage records and documentation.

Responsibilities

• Assist residents and families in adjusting to placement, illness, and the plan of care.
• Help residents with treatment wishes and transitions to the facility.
• Participate in resident and family council meetings and support admissions and discharge planning.
