Legacy Healthcare Financial Services LLC in Palos Heights, IL is seeking a Social Services Designee to join our caring team. You will help prepare residents for the future and ensure their social and emotional needs are met during their stay, coordinating discharges and contributing to resident care conferences.

The role requires at minimum a high school diploma or equivalent and two years of experience in a social work role, preferably in healthcare.

Qualifications

• As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor's degree in social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!

Responsibilities

• Assist in preparing residents for their future and ensure their social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Support residents to feel comfortable and safe during their stay.
