Job summary

Atrium Centers in Michigan is seeking a Social Services Director to lead the social services program, advocate for residents, and collaborate with care teams to ensure dignity and quality of life. You will oversee care planning, transitions, and regulatory compliance while supporting residents and families.

Qualified candidates are licensed social workers with geriatric experience, strong communication skills, and a compassionate, team-focused mindset.

Qualifications

• Licensed Social Worker is required.
• At least 1 year of supervised social work experience in healthcare, preferably geriatric.
• Knowledge of nursing home regulations, MDS, and reimbursement guidelines.
• Strong written and verbal communication skills.
• Compassionate, service-oriented mindset with residents and families.

Responsibilities

• Develop, plan, implement, and evaluate social services per guidelines.
• Identify and address residents' social, emotional, and psychological needs.
• Coordinate services for resident adjustment, care planning, and discharge.
• Collaborate with interdisciplinary teams for holistic, person-centered care.
• Support residents and families with resources and care transitions.
• Maintain documentation in compliance with regulatory standards, including MDS processes.
• Advocate for residents' rights and address concerns promptly and compassionately.
• Ensure compliance with Medicare, Medicaid, and other guidelines.

What You’ll Do

• Develop, plan, implement, and evaluate the Social Services program in accordance with federal, state, and center guidelines

• Identify and address residents’ social, emotional, and psychological needs

• Coordinate services to support resident adjustment, care planning, and discharge needs

• Collaborate with interdisciplinary teams to ensure holistic, person-centered care

• Assist residents and families with resources, education, and care transitions

• Maintain accurate documentation in compliance with regulatory standards, including MDS-related processes

• Advocate for Residents’ Rights and ensure concerns are addressed promptly and compassionately

• Support compliance with Medicare, Medicaid, and other reimbursement and eligibility guidelines

What You Bring

• Licensed Social Worker (required)

• At least one year of supervised social work experience in a healthcare setting, preferably working with geriatric populations

• Working knowledge of nursing home regulations, MDS procedures, and reimbursement guidelines

• Strong written and verbal communication skills

• Compassionate, service-oriented mindset with a desire to support residents and families

• Positive, dependable, and team-focused attitude
