Job summary

Atrium Centers is seeking a Social Services Assistant to support the development and execution of social service programs. You will help identify residents’ social, emotional, and psychological needs and coordinate service delivery with families and interdisciplinary teams.

The role emphasizes documentation, regulatory compliance, and a compassionate, resident-focused approach. Collaboration with care teams and adherence to guidelines are essential for success.

Qualifications

• Associate’s degree preferred, experience considered in lieu of degree.
• Experience in skilled nursing or long-term care preferred.
• Strong written and verbal communication skills.
• Ability to assist with documentation, assessments, care planning, and regulatory requirements.
• Compassionate, resident-focused approach with strong interpersonal skills.
• Organized and detail-oriented with ability to manage multiple priorities.
• Knowledge of resident rights, discharge planning, and psychosocial support preferred.
• Positive, can-do attitude with excellent attendance.

What You Bring

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)

• Experience working in a skilled nursing or long-term care setting preferred

• Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams

• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director

• Compassionate, resident-focused approach with strong interpersonal skills

• Organized and detail-oriented, with the ability to manage multiple priorities

• Working knowledge of resident rights, discharge planning, and psychosocial support services preferred

• Positive, can-do attitude with excellent attendance
