Job summary

PACS in Overland Park, KS seeks a Social Services Director/Coordinator to plan, develop, organize, implement, and evaluate our facility's social service programs, ensuring residents' medically related emotional and social needs are met.

You will work with administration, medical and nursing staff, develop policies, maintain progress notes, assist with discharge planning, and connect residents with Medicare/Medicaid information and other financial aid.

Qualifications

• High school diploma required; associate or bachelor’s degree preferred.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret professional documentation.

Responsibilities

• Plan, develop, and direct social service programs aligned with standards.
• Meet with admin, medical, and nursing staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review policies annually and propose changes.
• Identify medical and social needs of residents and plan accordingly.
• Coordinate social service activities with other departments.
• Assist residents in achieving higher self-care and well-being.
• Provide information on Medicare/Medicaid and financial assistance.
