Job summary

PACS seeks a Social Services Specialist to assist in planning, developing, implementing, and directing social service programs across the facility. You will work with administration, medical staff, and residents to meet medically related social needs.

The role emphasizes policy development, coordination with external agencies, and ensuring resident well-being and autonomy. A focus on confidentiality and compliance is required.

Qualifications

• Education/experience: high school diploma required; associate or bachelor’s degree preferred but not required.
• Healthcare experience preferred but not required.
• Ability to read, write, and interpret general policies and procedures.

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop and coordinate department policies and procedures.
• Review policies annually and suggest changes.
• Identify residents' medically related social and emotional needs.
• Coordinate with community agencies for resources and referrals.
• Assist residents in achieving independence and well-being.
• Provide information on Medicare/Medicaid and other financial programs.
• Maintain confidential resident care information and notes.
