Job summary

OVERLANDPARK seeks a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs, ensuring the medically related emotional and social needs of residents are met.

You will collaborate with administration, medical and nursing staff, review policies, coordinate services, assist with discharge planning, provide Medicare/Medicaid information, and maintain confidential resident records.

Qualifications

• Minimum high school diploma; associate or bachelor's degree preferred but not required.
• Healthcare experience preferred.
• Strong reading and writing skills; ability to interpret regulations.
• Ability to communicate effectively with managers and staff.

Responsibilities

• Assist Social Service Director in planning and directing social service programs.
• Meet with administration and staff for planning social services.
• Develop and implement policies for identifying residents' social and emotional needs.
• Provide Medicare/Medicaid information to residents and families.
• Maintain confidentiality of resident information and records.
• Coordinate social service activities with other departments.
• Assist with discharge planning and coordination with community agencies.
