Camino Ridge Post Acute in Mountain View, CA is seeking a Social Services Assistant. We’re willing to train the right candidate to assist the Social Services Director in planning, organizing, documenting, and evaluating patient needs to support emotional and social well-being.

The role includes answering phones, coordinating transportation, helping families find community resources, and providing Medicare/Medicaid information while maintaining HIPAA-compliant records.

Qualifications

• Previous experience in social services or skilled nursing preferred.
• Outstanding verbal and written communication skills.
• Ability to remain calm and professional in all work-related situations.
• Strong organizational skills with ability to multitask and protect confidentiality.

Responsibilities

• Assist the Director of Social Services in the day-to-day operation of the department.
• Answers phones, takes notes, coordinates transportation for patients/residents as needed.
• Supports patients and families in finding resources through research and relationships with community agencies.
• Provides Medicare/Medicaid and financial information when requested.
• Maintains accurate social service progress notes.
• Updates the social services log and directories as needed.
• Plans and organizes time and duties to efficiently maintain caseloads.
• Protects residents’ files and information per HIPAA requirements.
• Treats everyone with kindness and escalates issues professionally when needed.
