Camino Ridge Post Acute in Mountain View is seeking a Social Services Assistant. The role assists the Social Services Director in planning, organizing, documenting, evaluating, and ensuring patient needs are met timely to support and improve their emotional and social well-being.

The position emphasizes communication, confidentiality, and strong organization to manage caseloads and coordinate with community resources. Training is available for the right candidate.

Competenze

• Previous experience in social services or skilled nursing preferred.
• Outstanding communication (verbal and written) skills.
• Ability to remain calm and professional in all work-related situations.
• Strong organizational skills, ability to multitask and confidentiality required.

Mansioni

• Assists the Director of Social Services in the day-to-day operation of the department.
• Answers phones, takes notes, coordinates transportation for patients/residents as needed.
• Supports patients and families in finding resources to meet their needs through research, education, relationships with community agencies, etc.
