Job summary

Hanford Post Acute in California is seeking an Interim Social Services Assistant to support our Social Services Department on a full-time basis. The role involves addressing residents' medically related social and psychosocial needs and guiding families through discharge planning.

The ideal candidate has strong communication, confidentiality, and organization skills, with familiarity in Medicare/Medicaid and EHR systems.

Qualifications

• High school diploma or GED required.
• Associate or bachelor's degree in social work, human services, psychology, sociology, or related field preferred but not required.
• Healthcare or social services experience preferred.
• Familiarity with Medicare/Medicaid and discharge planning is a plus.
• Strong written and verbal communication skills.
• Ability to maintain confidentiality and handle protected information.

Responsibilities

• Assist the Social Services Director with planning, developing, implementing, evaluating, and directing the facility's social services programs.
• Identify and address residents' medically related social, emotional, and psychosocial needs.
• Assist residents in achieving the highest practicable level of independence and well-being.
• Provide emotional support to residents and families facing illness or loss.
• Gather and evaluate social information to support assessments and care planning.
• Participate in interdisciplinary care-plan meetings and resident conferences.
• Complete forms, reports, and progress notes promptly and accurately.
