Southern HealthCare Management is seeking a Social Services Assistant to support the social services function in our nursing center. You will welcome and admit new residents, complete psychosocial assessments, care plans, and discharge plans to ensure a positive experience while supporting treatment goals.

Monday–Friday dayshift; requires a bachelor’s degree in social work or a related human services field and at least one year of supervised social work experience in a healthcare setting;

Competenze

• Minimum high school diploma or equivalent required.
• Bachelor’s degree in social work or a related human services field required.
• One year supervised social work experience in a health care setting required.
• One or more years’ experience in health care education/ marketing / insurance preferred.

Mansioni

• Welcome and admit new residents and patients.

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
