Job summary

Horizon Health and Rehabilitation Center in Las Vegas, NV is seeking a Full-Time Social Services Assistant to join our team. The role involves completing initial assessments, guiding discharge planning, and helping families navigate Medicare/Medicaid and financial aid options.

Minimum high school diploma or GED is required; social work license is preferred but not required. Long-term care experience is advantageous; reliable transportation is a must.

Qualifications

• Must possess at minimum a High School Diploma or GED.
• Experience in long term care or skilled nursing facility is preferred.
• Social Work License preferred, not required.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

Duties & Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident
• Work closely with Case Management to coordinate patient discharges

Qualifications & Requirements

• Must possess at minimum a High School Diploma or equivalent G.E.D
• Social Work License preferred, not required
• Must have experience in long term care or skilled nursing facility
• Reliable transportation required
