Palm City Nursing & Rehab Center is seeking a Social Services Assistant to support the social service function in our long-term care and short-term rehabilitation facility. You will welcome and admit residents, complete psychosocial assessments, develop care and discharge plans, and help ensure a positive patient experience while treatments are implemented.

The role features a Monday–Friday dayshift schedule and a supportive team environment that emphasizes compassionate care and patient

Formación

• Minimum high school diploma or equivalent.
• Bachelor’s degree in social work or related human services field.
• One year supervised social work experience in healthcare.
• One year supervised social work experience in healthcare (duplicate).
• Experience in health care education/marketing/insurance preferred.
• Familiarity with medical terminology.
• Proficient PC skills.

Responsabilidades

• Welcome and admit new residents and patients.
• Complete psychosocial assessments and care plans.
• Develop discharge plans and coordinate services.
• Support Social Services Manager and ensure a positive resident experience.

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications:

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
