Job summary

Southern HealthCare Management in Boynton Beach, Florida, is seeking a dedicated candidate to support the social service function in a long-term care nursing center. You will work closely with the Social Services Manager to ensure positive experiences for new residents, completing necessary assessments and care plans.

The ideal candidate will have a High School Diploma, experience in social services or health-related fields, and familiarity with Medical Terminology. This role offers competitive benefits and a collaborative work environment.

Qualifications

High School Diploma or equivalent required.
Post-secondary education or work experience in a social service or health related field is a plus.
Must be familiar with Medical Terminology.
Must be proficient in the use of personal computers.

Responsibilities

Support the social service function in a long-term care and short-term rehabilitation nursing center.
Work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related to psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans.

Qualifications

High School Diploma or equivalent required.
Post-secondary education or work experience in a social service or health related field a plus (such as social services, social work, patient registration, care coordination, or discharge planning).
Must be familiar with Medical Terminology.
Must be proficient in the use of personal computers (Windows, Internet, and Applications/Programs navigation).
