Job summary

Empire Care Centers in Atlanta, GA is seeking a Social Services Assistant to develop and execute a comprehensive social services program for residents and families. You will assess medical-related social needs and coordinate family meetings with the treatment team.

The role advocates for residents' rights, supports discharge planning, and ensures compassionate service. Requires a Bachelor’s degree in Social Work and prior long-term care experience. Monday–Friday schedule in an on-site setting.

Qualifications

• Bachelor’s Degree or higher, preferably in Social Work.
• Prior experience working in a long-term care setting.
