Job summary

Southern HealthCare Management in Florida is seeking a Social Services Assistant to support the social service function within a long-term care and short-term rehabilitation nursing center. You will welcome and admit new residents and patients, and help complete psychosocial assessments, care plans, and discharge plans to ensure a positive experience.

You will work closely with the Social Services Manager to maintain high standards of care and service.

Qualifications

• Bachelor's degree in social work or a human services field (sociology, gerontology, psychology, etc).
• One year supervised social work experience in a health care setting.
• One or more years' experience in health care education/marketing/insurance preferred.
• Familiar with medical terminology.
• Proficient in using a personal computer.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments, care plans, and discharge plans.
• Support the social service function and collaborate with the Social Services Manager.
• Ensure a comfortable and positive experience while completing treatment plans.

You'll enjoy a Monday - Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
