We are seeking a dedicated and compassionate Social Services Assistant to join our interdisciplinary team full-time. In this role, you will help support our residents' psychosocial, emotional, and social needs to ensure they receive holistic, high-quality care throughout their stay.
Assist in identifying residents’ psychosocial, mental, and emotional needs.
Complete social histories and psychosocial assessments for new and current residents.
Collaborate closely with the interdisciplinary clinical team to advocate for and protect residents' rights.
Help maintain a safe, respectful environment by actively preventing and addressing resident concerns or grievances in accordance with facility policies and mandated reporting guidelines.
Assist residents and their families with transition planning, community resources, and daily care adjustments.
Complete required documentation accurately within the Electronic Medical Record (EMR) system.
Perform other related administrative and supportive duties as assigned by the Director of Social Services or Facility Administrator.
Specific Skills/Experience REQUIRED:
Experience in long‑term care, discharge planning, care planning, resident advocacy, and knowledge of state and federal regulations.
Skills/Experience PREFERRED:
Experience working in a skilled nursing facility, strong communication and organizational skills, and experience in working with residents, families, and interdisciplinary teams.
Technical & Compliance Requirements:
Proficiency with computers and Electronic Medical Record (EMR) systems.
Strong customer service background with a proven track record of positive resident, family, and community relations.
Must pass all mandatory Illinois state healthcare worker background check requirements.
Core Competencies:
Excellent communication and interpersonal skills with the ability to build rapport quickly with residents, families, and team members.
Strong organizational skills and the ability to work effectively both independently and as part of a collaborative team.
