Job description

Join to apply for the Social Service Worker role at Intermountain Health.

This is a full-time position with the pediatric crisis team at Primary Children’s Hospital in Salt Lake City. The crisis team provides evaluation and intervention to patients experiencing mental health crises in the emergency department and medical floors. The SSW has an important role in supporting the team and these patients/families with care coordination, discharge planning, and outreach calls post-discharge to ensure patients are able to access needed resources and supports.

Job Specifics

• Pay Range: $28.15 - $43.42 per hour
• Benefits Eligible: Yes
• FTE: Full time
• Shift: Monday-Friday, 8am-4pm

Job Essentials

• Organize and prioritize daily work by reviewing patient needs and complete documentation.
• Gather psycho-social information for patients and caregivers.
• Coordinate with care teams and outside agencies to ensure quality care and timely discharges.
• Act as an advocate for patients and families, ensuring effective communication.
• Provide initial intake, crisis intervention, and counseling services, including referrals.
• Identify patients at risk for delays, readmission, or complex needs and coordinate solutions.
• Support discharge planning from admission to departure, including developing discharge plans.
• Educate patients and families on Advance Directives.

Minimum Qualifications

• Bachelor's degree from an accredited institution
• Current state licensure as a Social Services Worker
• Computer skills (email, word processing, spreadsheets)

Preferred Qualifications

• Experience in a healthcare setting
