Job summary

Sunnyvale Gardens Post Acute is seeking a dynamic, compassionate, and experienced Social Services Director to lead our department. In this vital leadership role, you will plan, organize, and direct our social services programs to meet the medically related emotional and psychosocial needs of our residents.

Qualifications

• Education: Bachelor's degree in Social Work (BSW) or a human services field is required; Master's degree in Social Work (MSW) is highly preferred.
• Experience: Minimum of 2-3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.
• Leadership Skills: Proven experience managing a department or supervising staff within a healthcare environment.
• Technical Skills: Proficiency with electronic health record (EHR) software (e.g., PointClickCare) and regulatory documentation.

Responsibilities

• Department Leadership: Oversee the day-to-day operations of the Social Services Department, providing guidance and supervision to assistant staff.
• Care Planning & Assessment: Conduct comprehensive psychosocial assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
• Discharge Planning: Manage complex discharge planning processes, ensuring safe, seamless transitions of care back into the community or lower acuity settings.
• Regulatory Compliance: Maintain full compliance with federal, state (CDPH), and local regulations governing long-term care social services.
• Family & Resident Relations: Act as the primary liaison and advocate for residents and families, addressing grievances, rights concerns, and financial resource guidance (Medi-Cal/Medicare).

• Salary Range: $75,000 - $100,000 annually (depending on experience)
