Sunnyvale Gardens Post Acute is seeking a compassionate Social Service Assistant to support residents' emotional, social, and psychological well-being. You will work with the Social Services Director to advocate for residents and coordinate admissions and adjustments in a post-acute setting.

The role requires at least a high school diploma with preferred degrees in social work, psychology, or human services, and a minimum of one year in a skilled nursing or post-acute environment.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree preferred in Social Work, Psychology, or Human Services.
• Minimum 1 year of experience in a skilled nursing facility, post-acute care, or assisted living preferred.
• Excellent interpersonal skills with empathy and patience for elderly, medically fragile populations.
• Strong administrative skills, computer literacy, and ability to multitask in a fast-paced healthcare environment.

Responsibilities

• Resident Advocacy: Assist residents and families with admission, orientation, and adjustment to the facility.
• Discharge Planning: Help coordinate safe discharge plans, including home health care and community resources.
• Documentation: Maintain social service progress notes and assessments in compliance with regulations.
• Support Services: Identify and resolve psychosocial needs, liaising between residents, families, and clinical staff.
• Room Visits: Conduct regular one-on-one visits to ensure comfort and rights are respected.
