Job summary

Qualifications

• Bachelor's degree in Social Work (BSW) or related field required.
• MSW preferred.
• 2-3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.
• Leadership experience in healthcare settings.
• Proficiency with EHR software (PointClickCare) and regulatory documentation.

Responsibilities

• Oversee day-to-day operations of the Social Services Department and supervise staff.
• Conduct psychosocial assessments and develop individualized care plans.
• Manage discharge planning processes for seamless transitions.
• Ensure regulatory compliance with federal, state (CDPH), and local regulations.
• Liaise with residents and families regarding grievances and resources.
