Sunnyvale Gardens Post Acute seeks a dynamic Social Services Director to lead our department, planning, organizing, and directing psychosocial programs to meet residents’ emotional and psychosocial needs, and advocate for patient care in a collaborative healthcare environment.

This full-time role oversees daily operations, conducts assessments, manages discharge planning, ensures regulatory compliance with CDPH and other regulations, and serves as the liaison for residents and families regarding

Qualifications

• Bachelor’s degree in Social Work (BSW) is required; MSW preferred.
• 2–3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.
• Leadership experience managing a department or supervising staff in a healthcare environment.

Responsibilities

• Oversee day-to-day operations of the Social Services Department, providing guidance and supervision to assistant staff.
• Conduct comprehensive psychosocial assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
• Discharge planning and management of complex transitions of care.
• Regulatory compliance with federal, state (CDPH), and local regulations governing long-term care social services.
• Liaise with residents and families regarding grievances, rights concerns, and financial resource guidance (Medi-Cal/Medicare).
