Job summary

Sunnyvale Gardens Post Acute is seeking a compassionate Social Service Assistant to join our team. You will work closely with our Social Services Director to support residents' emotional, social, and psychological well-being.

Qualifications

• Education: High school diploma or equivalent required; associates/bachelors in relevant field preferred.
• Experience: Minimum of 1 year in skilled nursing facility, post-acute care, or assisted living preferred.
• Communication: Excellent interpersonal skills with empathy and patience for the elderly and medically fragile populations.
• Organization: Strong administrative skills, computer literacy, and multitask in a fast-paced healthcare environment.

Responsibilities

• Resident Advocacy: Assist residents and their families with admission, orientation, and adjustment to the facility.
• Discharge Planning: Help coordinate safe discharge plans, including arranging home health care, medical equipment, or community resources.
• Documentation: Maintain accurate, up-to-date social service progress notes and assessments in compliance with federal and state regulations.
• Support Services: Identify and resolve resident psychosocial needs, acting as a liaison between residents, families, and clinical staff.
• Room Visits: Conduct regular one-on-one visits with residents to ensure their comfort, preferences, and rights are being respected.

Sunnyvale Gardens Post Acute is seeking a compassionate, organized, and dedicated Social Service Assistant to join our team. In this role, you will work closely with our Social Services Director to support our residents' emotional, social, and psychological well-being. Compensation & Benefits

• Pay Range: $20.00 - $24.00 per hour (depending on experience)
• Job Type: Full-Time

Key Responsibilities
