Job summary

Sunnyvale Gardens Post Acute is seeking a compassionate Social Service Assistant to support residents’ emotional and psychosocial well-being under the Social Services Director. This on-site role focuses on advocacy and coordinating services.

The ideal candidate has a high school diploma (associates or bachelor’s preferred in Social Work or related) and at least 1 year of post-acute care experience, with strong communication and organizational skills.

Qualifications

• Education: High school diploma or equivalent; associate/bachelor’s in Social Work, Psychology, or Human Services preferred.
• Experience: Minimum of 1 year in a skilled nursing facility, post-acute care, or assisted living setting preferred.
• Communication: Excellent interpersonal skills with empathy and patience for elderly populations.
• Organization: Strong administrative skills and ability to multitask in a fast-paced healthcare environment.

Responsibilities

• Advocate for residents and families during admission and orientation.
• Coordinate discharge planning and connect residents with home health care or resources.
• Maintain accurate social service progress notes and assessments per regulations.
• Identify and address residents' psychosocial needs, liaising with families and clinical staff.
• Conduct regular one-on-one visits to respect residents' preferences and rights.

Social Service Assistant (Full-Time)

Sunnyvale Gardens Post Acute is seeking a compassionate, organized, and dedicated Social Service Assistant to join our team. In this role, you will work closely with our Social Services Director to support our residents’ emotional, social, and psychological well-being. Compensation & Benefits

• Pay Range: $20.00 – $24.00 per hour (depending on experience)
• Job Type: Full-Time

Key Responsibilities

• Resident Advocacy: Assist residents and their families with admission, orientation, and adjustment to the facility.
• Discharge Planning: Help coordinate safe discharge plans, including arranging home health care, medical equipment, or community resources.
• Documentation: Maintain accurate, up-to-date social service progress notes and assessments in compliance with federal and state regulations.
• Support Services: Identify and resolve resident psychosocial needs, acting as a liaison between residents, families, and clinical staff.
• Room Visits: Conduct regular one-on-one visits with residents to ensure their comfort, preferences, and rights are being respected.
