Qualifications

• High school diploma or equivalent required; an associate's or bachelor's in Social Work, Psychology, or Human Services is preferred.
• Minimum 1 year of experience in a skilled nursing facility, post-acute care, or assisted living is highly preferred.
• Excellent interpersonal communication and empathy for the elderly and medically fragile populations.
• Strong organizational skills and ability to multitask in a fast-paced healthcare setting.

Responsibilities

• Resident Advocacy: Assist residents and their families with admission, orientation, and adjustment to the facility.
• Discharge Planning: Help coordinate safe discharge plans, including arranging home health care, medical equipment, or community resources.
• Documentation: Maintain accurate, up-to-date social service progress notes and assessments in compliance with federal and state regulations.
• Support Services: Identify and resolve resident psychosocial needs, acting as a liaison between residents, families, and clinical staff.
• Room Visits: Conduct regular one-on-one visits with residents to ensure their comfort, preferences, and rights are being respected.
