Fair Oaks Health & Rehabilitation in Fairfax, VA is seeking a compassionate Social Services Assistant to support residents and families, addressing social, emotional, and practical needs as part of an interdisciplinary team.

You will promote dignity and quality of life, provide emotional support, and help residents access community resources while maintaining confidentiality and respect in all interactions.

Qualifications

• Bachelor's degree in Social Work, Human Services, Sociology, Psychology, Special Education, Rehabilitative Counseling, or a related field preferred.
• This is a non-licensed position.
• Experience in a healthcare, long-term care, social services, or human services setting preferred.
• Strong communication, organization, and interpersonal skills.
• Compassionate and respectful approach when working with residents and families.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Interact with residents and families while maintaining dignity, respect, and confidentiality.
• Provide emotional support and assistance to residents and families.
• Build positive relationships between residents, families, and staff.
• Promote resident individuality, independence, and choice.
• Assist residents and families with identifying and accessing appropriate community resources.
• Participate in resident care planning and interdisciplinary team meetings.
• Document social services interactions and resident needs accurately.
• Assist with gathering social histories and information about residents' backgrounds.
• Observe and report changes in residents' behavior, attitude, mood, or social needs.
• Advocate for residents and help ensure their concerns and needs are communicated to the appropriate team members.
• Assist residents and families in navigating resources and services available to them.
