Job summary

Francis Marion Manor Health & Rehabilitation in Marion, Virginia is seeking a compassionate Social Services Assistant to join our care team on a full-time basis. You will support residents and their families with medically related services, grievances, or placement needs while helping to ensure the highest quality of care.

Key duties include coordinating with the Social Services Department, maintaining confidentiality of protected health information, and assisting with safe discharge planning

Qualifications

• 1 year of supervised social work experience in healthcare.

• Assist the Social Services Department.
• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.

Social Services Assistant Position Summary
