Job summary

Aston Group is seeking a compassionate Social Services Assistant to join our skilled nursing facility team in Florida. This role supports the Social Services Director in planning, developing, implementing, and evaluating services to enhance residents' quality of life and well-being.

You will work closely with residents, families, and other departments, assist with counseling and support services, maintain records, and help ensure compliant, patient-centered care within a caring culture.

Qualifications

• Bachelor's in social work preferred or human services degree accepted
• Previous experience in the field required; long-term care experience preferred
• State regulations may require certain certifications/licensures

Responsibilities

• Assists with planning and implementing a comprehensive Social Services program for residents and their representatives
• Identifies social needs and maintains records related to residents' social work needs and care
• Consults with the Director of Social Services and other departments regarding interdisciplinary issues and record-keeping
• Works directly with residents and their representatives facing personal or environmental challenges linked to health conditions

Monday to Friday

We are seeking a compassionate and dedicated Social Services Assistant to join our skilled nursing facility team. The Social Services Assistant will support the Social Services Director in providing high-quality care and services to our residents, helping to enhance their quality of life and overall well-being. The Social Services Assistant assists the Director of Social Services in the planning, developing, organizing, implementing, and evaluating our customers.

Social Services Assistant Responsibilities:

• The Social Services Assistant assists with planning and implementing a comprehensive Social Services program which provides for counseling and other support services for customers and legal representatives in the Center.
• Identifies medical-related social needs of customers, provides appropriate services to meet the individual, as well as collective needs of customers, and maintains records relating to the customers’ social work needs and care.
• Consults with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of appropriate records.
• Works directly with customers and legal representatives experiencing personal and environmental difficulties or concerns related to the customer’s physical or emotional condition.

Social Services Assistant Qualifications:

• Bachelors in social work is preferred. A bachelor’s degree in human services may also be considered.
• Previous experience in the field required. Previous long term care experience strongly preferred.
• Needs to possess any Certifications/Licensures required by State regulations.
