Job summary

National Youth Advocate Program seeks a SOAR Representative in Homewood, IL to help eligible adults access Social Security disability benefits. The role focuses on those experiencing homelessness or at risk, with mental health or co-occurring conditions.

You will collaborate with families, caseworkers, and agencies to build care plans, promote recovery, and ensure timely documentation and training completion. NYAP values advocacy and service impact.

Qualifications

• Bachelor’s degree in human services related field or CRSS certification.
• SOAR certified within 30 days of hire.
• Reliable telephone.
• Proficient use of word processing, spreadsheets and database programs.

Responsibilities

• Promote recovery by increasing access to SSI/SSDI for individuals with disabling conditions.
• Develop individual treatment/service plans with client, caregiver, and caseworker.
• Maintain client contact and review goals with supervisor.
• Complete required trainings and documentation on time.
• Engage with individuals to meet service needs and goals.
