Job summary

Kenneth Young Center in Illinois is seeking a full-time SOAR Benefits Specialist to help adults transition from nursing facilities access Social Security benefits and other entitlements. You will evaluate eligibility, gather medical records, process applications, and coordinate with Social Security staff while maintaining accurate documentation.

The role requires a BA/BS in Social Work or related field, a valid Illinois driver's license, reliable transportation, and strong teamwork.

Qualifications

• BA/BS in Social Work or closely related discipline from an accredited institution of higher learning.
• Valid Illinois driver’s license, auto insurance, and access to reliable vehicle.
• Basic technology, smartphone, and computer proficiency; knowledge of web-based team communication tools (ex. Microsoft Teams), electronic health records, telehealth programs, and internet skills.
• Preferred experience working with adults with serious mental illness and/or adults with other complex physical health and mobility needs.
• Employee must complete SAMHSA’s online SOAR training within 45 days of being hired.
• Role involves community travel and occasional Work from Home with approval.

Responsibilities

• Complete SSDI/SSI Outreach, Access and Recovery (SOAR) training within 45 days of hire and maintain knowledge of best practices.
• Assess client eligibility and complete applications for entitlements.
• Maintain a caseload of clients applying for benefits and monitor appeals.
• Gather medical history and submit records to facilitate benefits applications.
• Complete SSI/SSDI benefit applications per SOAR process and enter into the SOAR system.
• Explain and coordinate representative payee services as needed.
• Assist clients with Medicaid spenddown requirements and related paperwork.
