Job summary

Alexander Youth Network is seeking a SOAR Case Manager to work on-site at My Sister Susan’s House (MSSH) and support clients in HEARTH programs. You will assist with day-to-day case management, help residents apply for mainstream benefits, and guide youth toward education and employment goals.

The role focuses on coordinating housing and individual goals, ensuring access to Medicaid and other public assistance, with a commitment to the agency’s mission and policies.

Qualifications

• Minimum of 21 years of age.
• Bachelor’s degree from four-year college or university; or one to two years’ experience and/or training; or equivalent combination of education and experience.
• Valid North Carolina Driver’s License.

Responsibilities

• Provide services for clients in the MSSH, HEARTH TLP and HEARTH Beyond Program by assisting with day-to-day case management.
• Assist young adults in applying for mainstream benefits and health insurance coverage.
• Support youth to develop goals towards education, employment, independent life skills and self-sufficiency.

The SOAR Case Manager is responsible for assessing clients’ eligibility for SOAR services, collecting, and summarizing disability documentation, and submitting final applications to access mainstream benefits on behalf of clients. This position will help ensure youth are accessing needed health insurance coverage through Medicaid and other resources, as well as other public assistance benefits according to their needs.
