National Youth Advocate Program seeks a SOAR Representative in Homewood, IL to help eligible adults access Social Security disability benefits. The role focuses on those experiencing homelessness or at risk, with mental health or co-occurring conditions.
You will collaborate with families, caseworkers, and agencies to build care plans, promote recovery, and ensure timely documentation and training completion. NYAP values advocacy and service impact.
The SOAR Representative works to increase access to Social Security disability benefits for eligible adult individuals who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or co-occurring substance use disorder.
Location: Homewood, Illinois
Hours: Monday - Friday, 3pm - 11pm
