Qualifications

Responsibilities

• Engage in street outreach to assist individuals experiencing homelessness with income, housing, and services.
• Provide in-reach to inpatients to connect them with SSI/SSDI benefits.
• Collect data for SSI/SSDI applications and coordinate with SSA, hospitals, and HMIS systems.
• Maintain documentation in EMR/OAT and complete required trainings.
