Job summary

Solaris HealthCare Coconut Creek is seeking an experienced MDS Coordinator to join our skilled nursing facility in Coconut Creek, FL. The ideal candidate will have SNF MDS experience and a valid RN or LPN license in Florida, CPR certification, and a solid understanding of OBRA and Medicare/Medicaid PPS processes.

The MDS Coordinator will support the Director of Nursing by ensuring timely and accurate MDS, CAAs, and care plans, coordinating RAI timings, and guiding staff through documentation

Qualifications

• Must have SNF MDS experience and solid MDS process knowledge.
• Strong communication and collaboration across care teams.
• Understanding of OBRA, Medicare PPS and care planning.

Responsibilities

• Assist DON with ensuring documentation meets Federal/State guidelines.
• Coordinate the RAI/MDS process from assessment to care plan.
• Ensure timely submission of MDS, CAAs and care plans.
• Provide guidance to staff on MDS documentation standards.
• Schedule and communicate care planning conferences and outcomes.
