Job summary

Solaris HealthCare Coconut Creek is seeking an experienced MDS Coordinator to support the Director of Nursing and ensure federal/state regulations are met for MDS/CAAs and care plans.

Responsibilities include coordinating RAI processes, ensuring timely and accurate documentation, and guiding the interdisciplinary team through PPS/OBRA workflows. Florida AHCA background screening is required; MDS experience is essential.

Qualifications

• Currently licensed as an LPN or RN in Florida; RN preferred; CPR certified.
• At least 1 year of MDS experience.
• Minimum 2 years of nursing in a Skilled Nursing Facility preferred.
• Strong knowledge of Medicare PPS, Medicaid reimbursement, OBRA, and MDS processes.
• Ability to lead and coordinate interdisciplinary teams.

Responsibilities

• RAI-MDS, CAA, Care Plan responsibilities.
• Coordinate the entire RAI process from MDS to care plan implementation.
• Monitor documentation for accuracy and timeliness.
• Schedule and communicate resident care planning conferences.
• Ensure validation, proper signatures, and compliance with OBRA/ PPS rules.
