Job summary

Solaris HealthCare Coconut Creek, LLC in Florida is seeking a qualified MDS Coordinator to support the Director of Nursing. You will ensure MDS, CAAs, and Care Plans meet federal and state guidelines, coordinate the RAI process, and monitor documentation accuracy and timeliness.

Join our team to lead MDS activities, support interdisciplinary care planning, and ensure compliant PPS and OBRA practices, with emphasis on accurate assessments and communication across staff and residents.

Qualifications

• Requires MDS experience and knowledge of OBRA regulations
• Nurses with SNF experience preferred
• CPR certification and active Florida license required

Responsibilities

• Assist DON with ensuring documentation meets Federal, State, and Certification guidelines.
• Coordinate the RAI/MDS process from assessment through care plan implementation.
• Monitor and track MDS timelines, accuracy, and completeness.
• Coordinate resident data collection and interviews to ensure standard practice.
• Facilitate timely and accurate MDS submissions to State and Federal agencies.
• Instruct staff on RAI processes and PPS-related tasks.
• Support communication about changes in PPS timelines.
