Job summary

Solaris HealthCare Forest Lake in Apopka, FL is seeking a dedicated MDS Coordinator to support the Director of Nursing. You will ensure documentation aligns with Federal, State, and Certification guidelines and coordinate the MDS, CAAs, and Interdisciplinary Care Plan with the team.

Requirements include a nursing degree and an active Florida license, plus at least two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility.

Qualifications

• Requires current, unencumbered Florida nursing license and nursing degree.
• Experience with MDS 3.0, RAI and CAAs is essential.
• Strong teamwork with the IDT and ability to coordinate care plans.

Responsibilities

• Assign sections of the MDS to qualified staff.
• Coordinate RAI process and certify assessments.
• Attend care planning and enter care plan decisions.
• Assist with month-end duties and other assignments as needed.
