Job summary

Solaris HealthCare Coconut Creek is seeking a skilled MDS Coordinator to support the Director of Nursing and ensure all documentation meets Federal, State, and Certification guidelines. You will coordinate the entire RAI process, guaranteeing accuracy, timeliness, and completeness of the MDS, CAAs, and Care Plans.

The role requires LPN or RN licensure in Florida (RN preferred), CPR certification, and a minimum of 1 year of MDS experience with strong leadership and communication abilities.

Qualifications

• At least 1 year of experience in MDS.
• Minimum 2 years of nursing in a Skilled Nursing Facility preferred.
• Strong knowledge of Federal Medicare PPS and Medicaid reimbursement.
• Knowledge of OBRA regulations and the MDS process.
• Demonstrated leadership and excellent communication skills.
• Highly organized with acute attention to detail.

Responsibilities

• RAI-MDS, CAA, Care Plan responsibilities and implementation.
• Ensure accuracy and timeliness of MDS, CAAs, and Interdisciplinary Care Plans.
• Coordinate MDS assessments and care plan schedules with the interdisciplinary team.
• Monitor documentation standards and obtain required signatures.
• Submit MDS data to State and Federal agencies; ensure compliance with OBRA and PPS.
• Lead and train staff on the RAI process and related systems.
